| Sponsor Commitment Form | This form must be completed if another person will be paying for your studies at the ESL Academy. The person paying needs to complete and sign the form. | Under 18 Waiver Form | Students under the age of 18 must submit this form prior to an admission decision. This form must be completed and signed by the parent of the student who is under 18 and the designated in-country guardian. | Download Form |
| Transfer In Form | This form is for student who wish to transfer from another school in the US to the ESL Academy. Students should complete the top section and give the form to the DSO at the current school. The DSO will complete the second portion and email the completed form to the ESL Academy. We only accept student in active status and in good standing at their current school. An I-20 cannot be created until the student's SEVIS record is released to our school.
