All CU Denver community members are required to fill out this COVID-19 self-report form if they:

Moderna, Pfizer Booster Shots Now Available at Health Center at Auraria
The Health Center at Auraria is now offering free COVID-19 booster shots and flu shots. The Johnson & Johnson vaccine is NOT available at the Health Center. However, the CDC has approved mix and match protocols that permit the use of either Moderna or Pfizer vaccine as an acceptable booster for those who initially received the J&J vaccine. Gov. Polis signed an executive order today making all Colorado adults eligible to receive booster shots. However, you must have received your second dose of Moderna or Pfizer at least six months ago or your J&J vaccine at least two months ago.
Flu shots are limited this year; therefore, all flu shots will be administered at the same time an individual either receives a COVID-19 booster shot or a first or second dose of the COVID-19 vaccine.
Booster shots and flu shots will be administered in the Plaza Building, room 111, Monday – Thursday, 9 a.m. – 5 p.m., and Friday, 9 a.m. – 3 p.m. To reduce wait times, appointments are required. Use this link to schedule an appointment. You must bring a copy of your vaccine card to your booster shot appointment. If you do not have a vaccine card, click here and print out a copy of your vaccine record from the State of Colorado’s immunization data base.
Children Ages 5 – 11 Now Eligible for Vaccine
The U.S. Food and Drug Administration and CDC have approved final emergency use authorization of the Pfizer-BioNTech COVID-19 vaccine for children ages 5 to 11. While the Health Center at Auraria does not offer vaccines to children, we encourage campus community members to take their children to an upcoming vaccine clinic or mobile vaccine clinic.
