Graduate Course Substitution - For graduate students to obtain approval to take courses not listed on the MS/MA curriculum checklists.

Faculty Academic Advisor/Graduate Degree Plan Dual form for graduate students to (1) Establish a Faculty Academic Advisor and (2) Create a degree plan. This form is due on the last day of classes of the student's first semester in the program.
