• Teacher Contact Information (if you are participating through school)
• School Address and phone number
• Team Name (if participating as a Team)
• Project Title
• Project Abstract (limit 250 words)
• Project Research Plan
• Form 1: Checklist for Adult Sponsor
• Form 1A: Student Checklist
• Form 1B: Approval Form (for each student researcher)
• Form 3: Risk Assessment Form
• Any Additional Forms required for your project (how you answer questions during registration will determine which forms will be required. This will show during registration. You may also use the ISEF Rules Wizard for guidance.)
• All student participants must fill out the DMRSEF Participant Waiver at least once during the 2023-2024 fair season

• You must designate a Team Captain
• The Team Captain is responsible for submitting all required ISEF Forms for the team's project via this registration form
• Each Team Member must also complete this registration form, but will only be prompted to submit ISEF Form 1B
• Each Team Member (including the Captain) should be careful to use the same Team Name
• Each Team Member (including the Captain) must fill out the DMRSEF Participant Waiver at least once during the 2022-2023 fair season

• Please use the following naming convention for each form: 
  • Student Last Name_ Student First Name _ Form Name.
  • EXAMPLE: Smith_Jane_Form1B

• Please limit individual file sizes to 5MB (at most!). We recommend using https://www.ilovepdf.com/compress_pdf to compress PDF file size.
